AVIVA Membership Program FAQs

How does membership work?

It’s fast and easy to set up! We like to verify a membership plan is right for you before you commit to one. You can fill out a membership application form above or we can sign you up over the phone or when you come into our office. Once you’re officially an AVIVA member, we’ll set you up with a practitioner to make a personalized plan based on your specific needs.

Who is an AVIVA Membership Plan for?
Membership plans are for clients who use regular services at AVIVA and would like more personalized (and cheaper!) way to access these treatments. We have comprehensive plans for our wellness services, our hormone therapy services, and our skin care services.
How do I cancel a membership?

We hate to see you go, but we understand that circumstances change and you may need to cancel your membership. We make it just as easy to cancel as it was to set up. Please give us at least 15 days cancellation notice before your next monthly payment is due. Simply call our office or come in and we will walk you through the cancellation process.

Should I tip my aesthetician or medical practitioner?

Nope! The best tip you can offer us is your repeat business.

Do you accept insurance or HSAs?

We do not work with any insurance group.
However, we do accept HSA and FSA Cards for covered health services (mostly services that aren’t cosmetic but check with your plan first!)

What payment methods do you accept?

We accept cash and all major credit cards: Visa, MasterCard, American Express, and Discover.

Do I need to schedule an in-office visit or do you offer virtual visits?

Depending on the service, we can do both.

We are happy to invite you to our office located in Millcreek, UT for your initial consultation OR we can schedule a virtual visit.

Certain services are available virtually, and some follow ups can be conducted virtually as well.
