Hormone Health Subscription Plan Timeline
Day 1
First Steps: After initial, comprehensive lab results have been received, an initial in-depth evaluation will educate you on available hormone treatment options and other wellness recommendations.
Weeks 8-9
Labs: Follow up labs need to be ordered and drawn before your next appointment. These labs help determine if your plan is on track or needs adjustments.
Week 11
First Follow-Up: Time to review your lab results! This visit will assess your progress feedback and lab results. Your provider will make adjustments to your program and prescriptions as needed.
Months 6-12
Quarterly Appointments: Quarterly check-ins to review progress, discuss overall wellness goals and address any symptoms. Make prescription adjustments as needed and refill the next 90 days. Shortly before your annual, we will discuss labs.
Months 13-14
Labs: Please get your yearly labs completed at a LabCorp near you. Our team should have emailed you your electronic lab order. These need to be drawn BEFORE your next visit.
Month 15
Yearly Review: Time to review your blood tests and symptom checklist! This visit will plan out the next 12 months of wellness goals.
Months 18-24
Quarterly Appointments: Quarterly check-ins to review progress, goals and any symptoms that are still lingering. Make prescription adjustments as needed and refill the next 90 days.

How does membership work?
It’s fast and easy to set up! We like to verify a membership plan is right for you before you commit to one. You can fill out a membership application form above or we can sign you up over the phone or when you come into our office. Once you’re officially an AVIVA member, we’ll set you up with a practitioner to make a personalized plan based on your specific needs.
Who is an AVIVA Membership Plan for?
How do I cancel a membership?
We hate to see you go, but we understand that circumstances change and you may need to cancel your membership. We make it just as easy to cancel as it was to set up. Please give us at least 15 days cancellation notice before your next monthly payment is due. Simply call our office or come in and we will walk you through the cancellation process.

Should I tip my aesthetician or medical practitioner?
Nope! The best tip you can offer us is your repeat business.
Do you accept insurance or HSAs?
We do not work with any insurance group.
However, we do accept HSA and FSA Cards for covered health services (mostly services that aren’t cosmetic but check with your plan first!)
What payment methods do you accept?
We accept cash and all major credit cards: Visa, MasterCard, American Express, and Discover.
Do I need to schedule an in-office visit or do you offer virtual visits?
Depending on the service, we can do both.
We are happy to invite you to our office located in Millcreek, UT for your initial consultation OR we can schedule a virtual visit.
Certain services are available virtually, and some follow ups can be conducted virtually as well.
