How do I join ASA?
Fill out our application form below to join the AVIVA Savings Account
We’ll contact you to discuss the program. At that point, you choose the amount you would like to save each month and the date you would like to have the funds withdrawn
Once your account is set up, each month we deposit your chosen amount into your ASA
When you are ready to make a purchase we deduct the amount from your ASA and apply it to the service, lab test, or retail item. If you still have a balance on your ticket, you pay the remaining balance

How does an ASA work?
It’s fast and easy to set up! We like to verify an ASA is right for you before setting up the account. You can fill out an ASA application form above or we can sign you up over the phone or when you come into our office. You choose the amount you would like to save each month and the date you would like to have the funds withdrawn. It’s that simple! Once your account is set up, each month we deposit your chosen amount into your ASA. When you are ready to make a purchase we deduct the amount from your ASA and apply it to the service, lab test, or retail item. If you still have a balance on your ticket, you pay the remaining balance.
Who is an ASA for?
How do I cancel an ASA?
We hate to see you go, but we understand that circumstances change and you may need to cancel your subscription. We make it just as easy to cancel as it was to set up. Please give us at least one week’s cancellation notice before your next monthly payment is due. Simply call our office or come in and we will walk you through the cancellation process. If you have a remaining balance in your ASA, you are welcome to spend the funds on any service or retail item. We will even allow you to utilize the perks for your last transaction!
Should I tip my aesthetician or medical practitioner?
Do you accept insurance or HSAs?
However, we do accept HSA and FSA Cards for covered health services (mostly services that aren’t cosmetic but check with your plan first!)

Do I need to schedule an in-office visit or do you offer virtual visits?
We are happy to invite you to our office located in Millcreek, UT for your initial consultation OR we can schedule a virtual visit.
Certain services are available virtually, and some follow ups can be conducted virtually as well.
